Release of Liability and Program Policies

RELEASE OF LIABILITY AND PERMISSION TO PARTICIPATE IN ACTIVITIES

  • I understand that I may be enrolling my child in a program that is a) offered by the company, STEM For Kids, LLC, or b) offered by an independent entity operating as a STEM For Kids Franchise, or c) offered by a trained instructor or an independent operator that is not affiliated with STEM For Kids LLC or SFK Franchising Inc ("Independent Operator").
  • I understand that even with proper supervision, the use of the equipment and program activities have some risk, and therefore the program, its employees are not liable for risk. I hereby consent to my child’s participation in the STEM For Kids Programs and do forever release, acquit, discharge, and covenant to hold harmless STEM For Kids LLC, SFK Franchising Inc, their affiliates and franchisees, and their respective owners, officers, employees, agents, affiliates, representatives, and assigns (collectively "STEM FOR KIDS"), and the Independent Operators from any and all actions, causes of action, and claims that may arise on account of, or in any way growing out of, directly or indirectly, all known and unknown personal injuries or property damage which I as a parent or legal guardian or my child may have or hereafter acquire as a result of my child’s participation in the STEM For Kids Programs.
  • I hereby grant permission for the program administration to take necessary steps to obtain emergency medical care, if warranted in the opinion of the program administration. These steps may include the following:
    • attempts to contact parent or guardian,
    • attempts to contact persons listed on the registration form.
    • If I or the persons listed cannot be reached, the program administrator will call the paramedics or an ambulance, if so warranted in the opinion of the program administration.
    • I understand and agree that any expense incurred will be my responsibility.

Media Release

  • I hereby authorize and grant full permission to STEM For Kids to publish and copyright all photographs, videos,s and other media in which my child appears while enrolled in a STEM For Kids program. I agree that STEM For Kids may use, in whole or in part, photographs, video, writings, drawings, creations, voice recordings of program participants for activities including but not limited to publications, websites, staff training, curriculum development, marketing, and promotions, broadcasting of reproductions over the radio, television, and on the Internet (including social networking). I affirm that I have the legal right to grant this permission. If such is not consent to, I will indicate so when registering for a program. I understand that I will not receive any monetary compensation now or in the future. I do hereby release and hold harmless STEM FOR KIDS from any claims.
  • If I am enrolling my child in an online class/camp/program, I understand that my child may be on a live video conference. I understand that the live conference may be recorded and may contain audio and video interactions that include my child.

Medical Policy

  • I understand and agree that STEM For Kids employees, agents and representatives, and the Independent Operators are not authorized to store any prescribed medications or to administer any medical treatments to my child.

Participant Code of Conduct

In order for all campers to have a safe and enjoyable time, participants must demonstrate good behaviors. Parents must review the following behavior expectations and consequences with their child prior to the start of any STEM For Kids program. I agree that I understand the behavior expectations and consequences and that I have reviewed these with my child.

  • Participants must take responsibility for his/her actions and abide by program rules and policies.
  • Participants must treat the counselors, instructors, and fellow program participants with respect, kindness, and honesty.
  • Participants must listen attentively to counselors and instructors.
  • Participants must show support and compassion for other participants by listening, encouraging, and helping them during program activities.
  • The participant should be fair when conducting any activities or playing games; show good sportsmanship.
  • The participant should ask counselors or instructors for help if he/she feels unable to meet these expectations.
  • Participants must always try his/her best and have fun in ways that are safe for them and others.
  • Except when excused by the program instructor or counselor, all participants are expected to remain at their designated area.

 

Should a negative behavior issue arise, STEM For Kids and the Independent Operators will apply the following guidelines:

  • First Occurrence: A quiet, verbal reprimand
  • Second Occurrence: Verbal warning to the participant and a phone call to parent or guardian
  • Third Occurrence: The parent or guardian will be notified in writing via email.
  • If the inappropriate behavior continues beyond the written warning, the participant will be asked to leave for the remainder of the program with no refund of already collected fees.
  • If the offense is severe including, but not limited to, fighting, theft, vandalism, possession of weapons, alcohol, cigarettes, or illegal substances, inappropriate language, severe verbal threats, physical aggression, leaving the designated area without permission, or sexual misconduct, the participant will be immediately dismissed from the current program and any other programs he or she is registered to attend, bypassing all steps above, and NO REFUND will be given. The actions taken will be at the discretion of the STEM For Kids program director.
  • There will be no refunds if a program participant is temporarily or permanently suspended or dismissed from a program due to failure to follow the code of conduct.

Terms of Website Use

  • The usage of this website together with all online resources provided by STEM For Kids is governed by the Website Terms of Use as found here and updated from time to time.

COVID-19 LIABILITY WAIVER

  • In accordance with the guidance and protocols issued by the World Health Organization (WHO), the Centers For Disease Control and Prevention (CDC), and many local authorities for slowing the transmission of COVID-19, I agree, represent, and warrant that neither I nor the participant shall participate in programs (other than an exclusively online program and services) of STEM For Kids within 14 days after (i) returning from highly impacted areas subject to a CDC Level 3 Travel Health Notice, (ii) exposure to any person returning from areas subject to a CDC Level 3 Travel Health Notice, (iii) exposure to any person who has a suspected or confirmed case of COVID-19.
  • I agree to check the CDC Travel Health Notices list, which is regularly updated by the CDC at https://www.cdc.gov/coronavirus/2019-ncov/travelers/index.html, prior to participating in programs at STEM For Kids.
  • I agree, represent, and warrant that neither I nor the participants shall visit or utilize in-person programs of STEM For Kids if he or she (i) experiences symptoms of COVID-19 as per the CDC website: https://www.cdc.gov/coronavirus/2019-ncov/symptoms-testing/symptoms.html, including without limitation, fever, cough, or shortness of breath or (ii) has a suspected, diagnosed or confirmed case of COVID-19.
  • STEM For Kids franchisees and local centers have taken steps to implement recommended guidance and protocols issued by the Public Health Agencies for slowing the transmission of COVID-19 including but not limited to, the use restrictions as set forth above. I agree that STEM For Kids may revise its procedures at any time based upon guidance issued by the Public Health Agencies and further agree to comply with the STEM For Kids’ revised procedures prior to utilizing any programs.
  • I further acknowledge and agree that due to the nature of programs and services offered by STEM For Kids, the social distancing of 6 feet per person is not possible. I fully understand and appreciate both the known and potential dangers of utilizing programs of STEM For Kids and acknowledges that use thereof may, despite STEM For Kids’ reasonable efforts to mitigate such dangers, result in exposure to COVID-19, which could result in quarantine requirements, serious illness, disability, and/or death.
  • I, on my behalf and on the behalf of participating children, hereby forever release, acquit, discharge, and covenant to hold harmless STEM For Kids LLC, SFK Franchising Inc, their affiliates and franchisees, and their respective officers, employees, agents, affiliates, representatives, assigns and the independent Operators from any and all liability to me, my participating children and all personal representatives, assigns, heirs, next of kin of me or participating children for any loss, or an illness, or the death of me or participating children (or any person who may contract COVID-19, directly or indirectly, from me or participating children) whether caused by negligence, active or passive, of STEM For Kids or otherwise while I or my participating children are in or around any STEM For Kids program.

Pricing Policy

Payments

  • All payments are due in full at the time of registration and must be paid in full within one week of registration.

Transfer Policy

  • All transfer requests must be submitted in writing and may be sent to your local STEM For Kids representative via email or mail.
  • If the transfer is requested within 3 weeks before the program start date, a transfer fee of $25 per program per child will be charged to the customer. No transfers are provided for requests made on or after the program start date. Full tuition is forfeited.
  • Registrations can be transferred only to available camps or programs during the same calendar year.

Refund Policy

  • All refund requests must be submitted in writing and may be sent to your local STEM For Kids representative via email or mail.
  • $50 per program per child (subject to the maximum paid value of the program) is a non-refundable deposit and will not be returned for any reason. We don’t follow this policy.
  • For any registration (see exception for groups): A full refund less the non-refundable deposit will be provided until three weeks before the start of the program. After such time, tuition is forfeited and may be transferable to another program subject to the Transfer Policy.
  • For group registrations: No refunds are provided; your registration may be transferable to another week subject to availability.
  • Change Requests: Partial refund for reduced camp timing, like changing full day to half-day or reducing extended camp timing or cancellation of extended camp timing, will be provided if change requests are made in writing up until three weeks before the start of the program. After such time, no refund, credit, or transfer is provided.
  • No Shows: No refund, credit, or transfer is provided for a child who misses parts of the program or doesn’t come to the program for any reason.
  • If unforeseen conditions including but not limited to extreme weather conditions, outages, acts of terrorism, government regulations, epidemics, and other such circumstances outside of STEM For Kids' control ("UNCONTROLLED CONDITIONS") cause STEM For Kids to shut down its program for any length of time, no tuition will be refunded, credited or transferred. A 100% refund is provided only if a program is canceled by STEM For Kids management for conditions that are not UNCONTROLLED CONDITIONS.
  • There will be no refunds if a program participant is temporarily or permanently suspended or dismissed from a program due to a health condition or a failure to follow the code of conduct (see Code of Conduct above).

Children Departure at Program End

  • Participants must be promptly picked up at the scheduled program end time.
  • Pick-ups after 5 minutes of the slated departure time will incur late pickup charges at the rate of $1 per minute.
  • Any special instructions regarding children pick up or drop off after a program must be provided during registration.

Discounts and Promotions

  • Discounts may be offered from time to time per STEM For Kids pricing promotion. Discounts and promotional offers cannot be combined.

Communications Release

  • STEM For Kids may send newsletters, program, and camp information to me using the email address provided during registration. If such is not consent to, I will indicate so during program registration.

Program Guidelines

  • Participants are required to bring their own lunch and snacks. For camps, send in lunch (full-day and half-day campers) and a snack (full-day campers only) with your camper. We recommend sending in an extra snack if your child is signed up for extended camp timing.
  • Parents/guardians will be required to provide transportation to and from the program location.
  • Send participants in weather-appropriate clothing as we may have outdoor activities.

Covid19 Prescreening Agreement - IQRA

In order to keep our environment as safe as possible, we are asking that parents/guardians prescreen STEM for Kids Stateline program participants before they enter our building for STEM FUN programs.

Agreement:

When I send my child, ____________________, into a STEM for Kids Stateline  program at Rockford Iqra School I will prescreen them.  I can assure you of the following:

My child does not have a temperature of 100 0F or more.

My child is not on fever-reducing medicine specifically to reduce a fever.

My child has not had contact with or cared for someone with COVID 19 in the past 14 days to the best of my knowledge.

My child has not traveled outside the United States within the last 14 days.

My child has not been directed to self-quarantine by health officials.

My child is not experiencing and potential COVID symptoms at the time of drop off such as shortness of breath, chills, fatigue, muscle aches, headaches, loss of taste or smell, sore throat, nausea, vomiting, or diarrhea.
